The topical hyperbaric oxygen therapy debate.

چکیده
Oxygen Therapy Debate Dear Editor, In response to the article, “Topical Hyperbaric Oxygen and Electrical Stimulation: Exploring Potential Synergy” (Ostomy/Wound Management. 2002;48(11):42-50), by definition, hyperbaric oxygen therapy involves the entire body, increased atmospheric pressure, and the patient breathing 100% oxygen. The increased oxygen pressure and oxygen concentration help wound healing. The treatment provides one or more of the following effects: • Increased oxygen delivery to injured tissues • Infection control • Blood vessel formation (neovascularization) • Preservation of damaged tissues • Elimination of toxic gasses. It is important to note that the effects of hyperbaric oxygen therapy result from a systemic oxygenenriched blood stream and not from the oxygen’s direct contact topically. The patients described in the article were at normal atmospheric pressure, 14.7 psi at sea level (normobaric conditions), breathing only 20% oxygen. The application of topical oxygen is exactly that. Limb chambers are occasionally used for wound healing and these are not hyperbaric either. Kathryn Burghart, AHCRN Scottsdale, Ariz.

UHMS position statement: topical oxygen for chronic wounds.
A small body of literature has been published reporting the application of topical oxygen for chronic non-healing wounds . Frequently, and erroneously, this form of oxygen administration has been referred to as "topical hyperbaric oxygen therapy" or even more erroneously "hyperbaric oxygen therapy." The advocates of topical oxygen claim several advantages over systemic hyperbaric oxygen includi...
متن کاملTreatment of nervous system injuries by hyperbaric oxygen: a review of molecular mechanisms
Background and aims: The use of hyperbaric oxygen therapy is one of the treatment methods to treat many diseases and injuries such as decompression sickness, wound healing, carbon monoxide poisoning, thermal burns, and nervous system injuries. This article aims to review the possible mechanisms of the effectiveness of this treatment in neurological injuries according to the available studies...

Dear Editors, The recent article describing the use of the Delphi method to provide clinical guidance on the use of topical oxygen for chronic wounds (1) does not do justice to the Journal or enhance the science of wound care. The Delphi method relies on a panel of experts to answer questionnaires in rounds, revising their answers in light of the replies of other panel members with the goal of ...
متن کاملAdvances in the treatment of the diabetic foot: Is there a role for adjunctive hyperbaric oxygen therapy?
There have been many advances in the treatment of wounds made in the last decade. Innovative techniques of wound closure, topical agents, aggressive vascular repair, focused wound care management, and adjunctive hyperbaric oxygen therapy are but a few of these improvements. The vital role of oxygen in wound healing is becoming better understood, in no small part, due to Dr. T. K. Hunt and his c...
متن کاملDermal excisional wound healing in pigs following treatment with topically applied pure oxygen.
Hypoxia, caused by disrupted vasculature and peripheral vasculopathies, is a key factor that limits dermal wound healing. Factors that can increase oxygen delivery to the regional tissue, such as supplemental oxygen, warmth, and sympathetic blockade, can accelerate healing. Clinical experience with adjunctive hyperbaric oxygen therapy (HBOT) in the treatment of chronic wounds have shown that wo...
